Rules and regulations of loan service of oral
health education materials

|
The Oral Health Education Unit
has produced various oral health education materials
such as games, exhibits and models for free loan
service to local organizations.
|
|
2.
|
The loan service
of oral health education materials is operated on
reservation basis. |
|
3.
|
Education materials can be
reserved 5 days later within three months. The
loan period is up to 14 days.
|
|
4.
|
You can select
any of the following methods to reserve the education
materials:
|
i.
|
On-line reservation of
education materials is fast and convenient.
Responsible person of organizations can
enter our oral health education website,
the Toothclub, through www.toothclub.gov.hk
and register
as a recognized user. The organization will
then receive their user ID and password.
Users are requested to secure their user
ID and password in order to facilitate their
use of on-line services. Users may browse
our on-line oral health education material
loan service catalogue, select the required
materials and then reserve these materials
through the internet.

Users can visit our office
in person during office hours to select
and reserve materials.
|

iii.
|
Users can call the Oral
Health Information Hotline at 2713 6344
to request for the oral health education
material loan service catalogue. After the
user has selected the materials, they can
call our office at 2760 5225 during office
hour to reserve the requested materials
and book the date and time for collection
of the materials.

5.
|
Users should collect the reserved
materials within the allotted loan period. Should
users fail to collect their reserved material
within the period, the reservation will be cancelled.
|
|
6.
|
Please return the borrowed
material to our Unit on or before the due date.
If users cannot return their borrowed material
on the due date, please contact out staff in advance.
|
|
7.
|
If there is no other reservation
for the particular item, users may extend the
loan period for an additional 14 days. Users must
renew the borrowed items 3 days prior to the original
due date. Otherwise, the material should be returned
to the Unit on or before the due date.

8.
|
When collecting or returning
the borrowed materials, users are requested to
carefully check the materials. During the loan
period, users must ensure that the materials remain
intact, for example, refrain from putting
adhesive tapes on the exhibits. Should
there be any loss or damages on the education
materials, our unit will reserve the right for
any recourse actions.

Should users fail to adhere
to the above regulations when using the service,
our Unit will cancel the user's priority in using
the service.
|
|
10.
|
Since there is only a limited
number of education materials in stock, our Unit
reserves the right to alter the number of on-loan
materials without prior notice.
|

11.
|
Please bring your
own bags or approprate tools when collecting the
education materials. |
|
12.
|
Users can browse
their loan records anytime through the internet. |
|
13.
|
Our unit reserves all copyrights
to our education materials, and no duplication
is allowed.
|
|
14.
|
Our unit reserves
all rights to the on-loan education materials. |
|
